Patient Forms

If you are new to chiropractic, you might be anxious about what to expect from your first visit. We're sure you have many questions before visiting our Los Gatos chiropractic clinic. To help ease all your worries, here’s how your first chiropractic visit in our practice will unfold. Upon entering our office, our receptionist will welcome you as a member of our family. You will then be guided towards the New Patient Center, where you will be asked to complete the patient forms. This paperwork provides us with all the information we need to know regarding your health history or any preexisting medical conditions, and helps us prepare a chiropractic treatment plan, personalized for your particular situation.

Consultation

The next step you will take during your first chiropractic visit will be to have a consultation with Dr. Andrea, D.C., to discuss your health-related problems, concerns, and potential treatment options. 

During this initial appointment, Dr. Andrea, D.C. will ask you some questions which are meant to help her learn more about you, your condition, and your expectations. 

Based upon her findings, she can then determine how chiropractic care can meet your goals.

Examination

After your consultation, Dr. Andrea, D.C. will perform a complete chiropractic body exam, to test your reflexes and flexibility. 

It’s during the first chiropractic visit that other standard neurological, orthopedic, postural, or physical tests will be performed as well. 

However, rest assured that no procedure will be done in our office without your consent.

X-Ray Studies

Depending on your specific condition and the reason you chose to see a chiropractor, during your first visit we may take X-rays as well. 

X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. 

Most patients are amazed once they see their X-rays up close. 

In some cases, they can even identify their misalignments or degeneration themselves.

Report of Findings

Once your information is collected and all the examinations are performed, your chiropractor, Dr. Andrea, D.C. will give you a detailed report of all the findings she concluded upon your first visit. She will also answer any questions you might have, including:

 - How can a chiropractic adjustment help me find neck or back pain relief?

 - Can chiropractic restore range of motion?

 - Does chiropractic help muscle injury or joint pain?

 - How long following the first treatment will I start to feel better?

After reviewing your health history and goals, and upon examining your spine X-rays, Dr. Andrea, D.C. will most likely let you know her recommendations, and notify you whether your condition requires additional care from other providers. She will then provide you with the right treatment and the best wellness program for your needs.

Treatment

At the conclusion of this first visit, you can choose to experience your first chiropractic treatment. 

This may include spinal adjustments, physical therapy, and/or soft tissue massage. 

All of our chiropractic services and treatments are interactive, so you can feel free to express whatever concerns you might have about the different styles of treatment, anytime.

Wellness Program

Prior to leaving, your chiropractor will suggest a wellness program to incorporate outside of treatment. If you are experiencing pain when you arrive at your first chiropractic visit, the recommendations may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches. 

And if you desire, our wellness team can also work with you to help you create healthy habits and positive routines for your lifestyle. Since every person is unique, all our wellness plans are customized, and targeted towards helping you achieve good spinal alignment, and maintain a positive mental state.
